Because what a pregnant woman needs is a ‘friend’ in the real sense and not someone who will irritate her and make her feel worse by asking useless and seemingly nonsensical questions or saying the stupidest things ever.
Are you having twins?
Don’t look at how much weight your friend has gained and speculate that she might be having twins. Asking her something like that suggests that you mean that she has put on too much weight and of course, that is what you mean. Don’t think you hid your thoughts well in your question because you didn’t. Pregnant women are usually very sensitive when it comes to weight gain during pregnancy. Don’t make her feel worse by asking her something like that or insinuating that she has gained too much weight.

Why do you care whether it is an IVF or not? If she tells you that she is indeed having twins, don’t go asking if she went through IVF. Remember that IVF is not the ONLY way to have twins or multiples. Even normal pregnancies lead to twin births and multiple births. Of course you know that. Don’t ask her this unless you are the unexpected father.
How long do you plan on taking off work?
Really, she does not know and don’t trouble her with such questions. Currently she is struggling to get past morning sickness and constant urination. She will decide when the time comes.
